The increase in US measles cases is the big story in the mainstream media this week.  Case reports have tripled from 42 last year to 131 in 2008.  Apparently half or 65 cases involve unvaccinated children, many of whom were home schooled to avoid measles vaccination.  The other 65 contracted measles even though vaccinated against the virus.  The 65 unvaccinated cases suggest that parents have been avoiding the MMR (measles mumps rubella) vaccine over concerns that MMR could cause adverse events such as aseptic meningitis or autism.  The numbers also highlight the fact that vaccination does not always confer immunity from the virus, since half the reported measles cases received the vaccination.

Benefits of MMR (Measles, Mumps, Rubella Vaccination)

Dr. Bloch writes a glowing report about the benefits of MMR vaccine in 1985, Pediatrics, "in the first 20 years, it is estimated that vaccination against measles has prevented 52 million cases, 5,200 deaths, and 17,400 cases of mental retardation, achieving a net savings of $5.1 billion. "

Adverse Events from MMR, Three Live Viruses Combined

Compared to other vaccines, the CDC (Center for Disease Control) reports that MMR has the highest reported adverse event rate at 74.8 to 83.8 reports per million doses administered.   This may be due to the fact that all three vaccines which are combined in the MMR shot are live viruses (although attentated) injected into the child.  Measles virus has a very high affinity for brain tissue, and can be found in the brain even in normal healthy people 20% of the time.

Adverse events from injection of the live viruses in MMR reported by Dr. Demicheli of the Cochrane Review included thrombocytopenic purpura (low platelets), parotitis (inflammation of the salivary gland), joint and limb complaints, febrile convulsions within two weeks of vaccination and aseptic meningitis (related to mumps) (Urabe strain-containing MMR). 

Inadequate Adverse Event Reporting

Dr Demicheli concluded that the "reporting of safety outcomes in MMR vaccine studies,  are largely inadequate. The evidence of adverse events following immunisation with MMR cannot be separated from its role in preventing the target diseases." (link)

Dr Andrew Wakefield and the MMR Gut/Autism Link 

Measles Virus Electron MicrographDr Andrew Wakefield was the first to publish a study in Lancet in 1998 linking measles to autism.  Wakefield's 1998 study was later partially retracted in 2004, and finding of measles virus RNA in the gut of autistic children more fully contradicted by a 2008 study by Horning and Lipkin of the Mailman School of Public Health at Columbia University.

Apparently Wakefield's 1998 findings depended on PCR lab results from Dr John O’Leary, Unigenetics, which were found to be false positive due to lab error, later corrected after a lab inspection by Dr Bustin.  (Measles is an RNA virus, and the lab had inadvertently skipped the RT step needed to detect RNA). Even so, the Wakefield group still maintains there is a link between MMR and autism and their response can be viewed here.

Wakefield's work is often blamed for creating a lack of confidence in the MMR vaccine.  However, even before Wakefield's 1998 article, the MMR vaccine had already been linked to adverse events such as aseptic meningitis.  
 
Who to Believe? Dr. Wakefield or Your Pediatrician?

Dr. Hilton, in a 2007 article in BMC Public Health, wrote that parents are untrusting and suspicious of pediatricians' motives, which they felt are influenced by financial incentives.  Parents felt more credible sources of information were other parents and  Dr. Andrew Wakefield who was viewed as a whistle blower and champion of ordinary parents.

Measles Vaccines Banned in UK, Canada and New Zealand

Pluserix made by Glaxo SmithKline, and Immravax made by Aventis Pasteur are both measles vaccines.  The (urabe) measles vaccine was banned in Canada in 1990.  Two years later, in 1992, the two measles vaccines were banned in both the UK and New Zealand after vaccinated children developed aseptic meningitis. Japan Has Stopped Using MMR Combined Vaccine

Classic 4 day measles rashAccording to a BBC news report, Japan stopped using the MMR vaccine in 1993 when the MMR triple vaccine was shown to cause aseptic meningitis (one case for every  2026 recipients).  In Japan, the MMR has been replaced with three separate injections for measles, mumps and rubella.

MMR Monkey Studies Reported at IMFAR show link to Autism

A large study has been underway for 5 years at the University of Pittsburgh studying the effect of MMR vaccine in monkeys.

At the 7th Annual International Meeting for Autism Research (IMFAR), Dr. Laura Hewitson from the University of Pittsburgh, reports developmental delays, behavior and brain changes in macaque monkeys that mimic "certain neurological abnormalities of autism...There was a significant difference between the two groups. The vaccinated group had trouble developing reflexes, like holding things and the sucking reflex, which obviously gave them problems feeding...They also became more insular and more aggressive. There was also an increase in aggressive behaviour after they had their MMR vaccines, and they stopped exploring their surroundings as much."

MRI brain scans showed abnormal brain activity in the MMR vaccinated monkeys with increased opioid receptor activity.  Abnormal opioid receptor activity  has been linked to autistic behaviours. (link)

Japanese children are vaccinated later than in the UK. By the BBC's Charles Scanlon in Tokyo.  Japan's Health Ministry says the withdrawal of the MMR vaccine for children did not cause an increase in deaths from measles.  It followed UK Prime Minister Tony Blair's comments this week in which he cited Japan as an example of the dangers of not having the combined vaccine for measles, mumps and rubella.

There is a huge controversy in the UK, with thousands of parents refusing to let their child have the combined MMR injection over fears that it has links to autism and bowel disorders.  But the medical establishment says such a link does not exist, and that the MMR vaccine is far more effective than having separate injections.
Measles epidemics  Japan stopped using MMR because it was linked to outbreaks of non-viral meningitis and other damaging side-effects. Doctors say there were problems with the vaccine which was of a different type to that used in the UK.
They stopped using the MMR vaccine in 1993 and reverted to three separate injections for measles, mumps and rubella.

Japan's Health Ministry said more than 100 people died from measles over the next six years, while there were no deaths in the UK.  But a spokesman said even more had died from the disease during the period when MMR was being used.  He said the withdrawal of the triple vaccine had had no impact. Deadly epidemics of measles are far more common in Japan than the UK. The ministry says that is because children are often vaccinated much later. Japanese officials say they have no record of a connection between MMR and autism.

In 1989 a nationwide surveillance of neurologic complications after the administration of mumps vaccine was conducted in Japan, based on the notification of cases and the testing of mumps viruses isolated from cerebrospinal fluid for their relatedness to the vaccine by nucleotide sequence analysis. Among 630,157 recipients of measles-mumps-rubella trivalent (MMR) vaccine containing the Urabe Am9 mumps vaccine, there were at least 311 meningitis cases suspected to be vaccine-related. In 96 of these 311 cases, mumps virus related to the vaccine was isolated from cerebrospinal fluid. The unusually high incidence may have been partly a result of the adverse media publicity of the problem at the time of surveillance. We analyzed clinical features of 165 and 27 laboratory-confirmed mumps vaccine-related meningitis cases that occurred among the recipients of MMR and monovalent mumps vaccines, respectively, during a 1-year period after the introduction of MMR vaccine. The incidence of vaccine-related meningitis was similar among the recipients of MMR and monovalent Urabe Am9 mumps vaccines. Meningitis was generally mild and there were no sequelae from the illness. The complication was more frequent among male than among female children.

[Canada withdrew licences for the vaccine in May 1990. 

Two years later, on September 14, 1992 two measles vaccines (Pluserix & Immravax) were banned in the UK after it was revealed that children had developed meningitis. 

On Sept 16, 1992 the New Zealand government withdrew Pluserix. 

Five years later in 1997: the Smith Kline Beecham Urabe MMR jab was used in a mass vaccination programme in Brazil.  

The men involved on the vaccine committee that approved Pluserix included Salisbury, Professor McDevitt, DAVID HULL & Professor George Nuki, all connected to the persecution of Dr Wakefield.

The team regularly carried out MRI brain scans on both sets of monkeys and found the vaccinated animals showed abnormal brain activity. Their brains did not show the expected decrease in opioid receptors, the little cells which field opioids - a morphine like chemical linked to autism. The vaccinated monkeys had too many receptors which led to excess opioid activity. This activity has been linked with autistic like behaviours such as sleeplessness, hallucinations, lack of social skills, high pain threshold and constipation, all symptoms found in varying degrees in children on the autistic spectrum.

The study's principal investigator, Laura Hewitson from the University of Pittsburgh, reports developmental delays, behavior problems and brain changes in macaque monkeys that mimic "certain neurological abnormalities of autism."

The results are sure to be controversial, in part because they lend credence to studies first published in 1998 by British pediatric gastroenterologist Andrew Wakefield, one of Hewitson's co-authors on these findings.

This retrospective study (1979 to 1986) investigated the possible etiologic relationship between vaccination and aseptic meningitis in 115 hospitalized children who became ill within 30 days of vaccination with the Leningrad 3 strain of mumps virus and the Edmonston-Zagreb strain of measles virus. The etiologic viral diagnosis was based on serologic tests and the isolation of virus from cell cultures which distinguished between attenuated and "virulent" mumps virus. The incidence of mumps vaccine-associated meningitis was 1/1000 vaccine recipients. In 92% of children the incubation period was 11 to 25 days and 28% had associated swelling of the salivary glands. Sixteen cases (13.9%) had a positive cerebrospinal fluid culture (attenuated mumps virus, 6 cases; "virulent" mumps virus, 7 cases; echoviruses, 3 cases). Clustering of cases, seasonal occurrence and age of the patients suggested causal relationship with the vaccination in the majority of children. In 4 patients with attenuated virus isolation from cerebrospinal fluid the incubation period ranged from 17 to 20 days. Clinical findings did not differ from natural mumps meningitis. The course was uncomplicated and at discharge the patients had no sequelae. Measles virus was never found as a cause of the meningitis. The mumps vaccine virus should be recognized as one of the causative agents of aseptic meningitis in countries where less attenuated mumps vaccine is used.

The largest nationwide active surveillance of four Measles-Mumps-Rubella (MMR) vaccines was conducted in Japan. A total of 1255 pediatricians actively participated in the study, which comprised 8.6% of all members of the Japanese Pediatric Society. The total number of registered recipients of MMR vaccines was 38 203. They were arbitrarily given one of the MMR vaccines produced by three makers (Takeda, Osaka city, Kitasato Minato-ku. Tokyo and Biken Suita city, Japan) or the standard MMR vaccine made of designated strains (Kitasato's measles-AIK-C, Biken's mumps-Urabe Am9 and Takeda's rubella-To336) produced by Takeda, Kitasato and Biken and were observed for 35 days. The rates of virologically confirmed aseptic meningitis per 10,000 recipients were 16.6, 11.6, 3.2 and 0 for the standard MMR, Takeda MMR, Kitasato MMR and Biken MMR vaccines, respectively. The incidence of convulsions between 15 and 35 days was the highest with the standard MMR vaccine and the incidence of fever associated with vomiting occurring between 15 and 35 days (symptoms relevant to aseptic meningitis) were also the highest with the standard MMR vaccine. The incidence of parotid swelling was the lowest with Takeda MMR vaccine. This surveillance revealed that incidences of aseptic meningitis after administration of the standard MMR vaccine and of Biken MMR vaccine were different. This posed questions about the manufacturing consistency of the Urabe Am9 mumps virus vaccines. On the other hand, the National Institute of Health found that the biological characteristics of the Urabe Am9 mumps virus contained in the standard MMR vaccine and in the Biken MMR vaccine were different. The Biken Company reported that the mumps vaccine in the standard MMR vaccine was a mixture of two Urabe Am9 mumps vaccine bulks; one identical to that contained in the Biken MMR vaccine and the other produced by a different manufacturing process.

By means of RT-PCR, a portion of measles virus (MV) mRNA encoding nucleoprotein (NP) could be detected in 11 (18%) of 61 brain tissue samples obtained from administrative autopsy cases, who apparently had not suffered from subacute sclerosing panencephalitis (SSPE)-like central nervous system disorders. Most of the brain-derived NP sequences showed significant asynonymous nucleotide substitutions when compared with wild-type MV isolates and SSPE virus. Our present results suggest that MV commonly persists in the human brain without causing apparent clinical symptoms, probably due to decreased virus replication.
